Booking in advance is required. Please reserve the time through the government's 24-hour online booking system. Please also read the vaccination note before signing up.
有意接種者必須預約登記。請透過政府的24小時網上預約系統預約接種時段；登記前並請閱讀接種須知。
Documents to Bring 接種當日所需文件
- Identity document
- Consent form signed by parents/guardians
- Applicable to persons aged below 18
- Consent form can be downloaded from the Government’s website: www.covidvaccine.gov.hk
- For children who are 11 years old or below, apart from the consent form, they have to be accompanied by an adult family member, a domestic helper or a school teacher to receive vaccination
- SMS message or photocopy of booking confirmation
- 身分證明文件
- 已填妥的家長／監護人同意書
- 適用於未滿十八歲人士
- 同意書可於政府網站下載：www.covidvaccine.gov.hk
- 十一歲或以下的兒童，除同意書外，必須由一名成年親屬、家庭傭工或學校老師陪同接種疫苗
